Community Health Needs Assessment

Since the early 1990s, CentraCare’s hospitals have regularly assessed the changing needs of our communities and responded with appropriate programming and support for special projects. Since adoption of the Community Health Needs Assessment (CHNA) for not-for-profit hospitals included in the Patient Protection and Affordable Care Act (ACA) those activities have been formalized and coordinated across the hospitals of CentraCare Health.

The CHNAs for CentraCare’s six hospitals are presented, as required by the ACA and the Internal Revenue Service (IRS), individually for each hospital although the overall CHNA was conducted for the region served by CentraCare Health. The CHNA and related Implementation Strategies for each hospital were approved by the relevant hospital boards and are presented here for the communities we serve.
