Hospice is a special way of caring for people in the final stages of life. Hospice provides comfort and dignity for patients who have a six month or less life expectancy and are accepting of comfort care. CentraCare Health provides hospice care in a variety of settings through CentraCare Health Hospice and CentraCare Health - Sauk Centre.
Hospice provides emotional, spiritual, and physical support to terminally ill persons and their families.

Hospice Team
Hospice uses the combined specialties of a team of health care professionals to assist patients in the final stages of life. The focus of care is the patient and family as a unit.
Our hospice team includes:
- Patient’s physician
- Nurses
- Social workers
- Chaplain
- Bereavement
- Massage therapists
- Hospice volunteers
- Home health aides
- Rehabilitation therapists
Referrals
A referral to hospice can be made for all ages by the patient’s health care provider, the patient or family members. Family members and significant others assume primary responsibility for patient care.
Our service area includes communities within a 45-mile radius of St. Cloud and 30-mile radius of Sauk Centre.

Grief & Bereavement Services
Support during the grief journey
A bereavement coordinator, chaplain or hospice volunteers provide a variety of grief support to immediate family members for 13 months following the death of their loved one such as:
- Send cards or letters on special holidays or birthdays
- Call occasionally to assess how the family members are handling their grief
- Provide educational resources on grief
- Visit when needed to offer support

Hospice Special Programs
- We Honor Veterans is a pioneering campaign developed by the National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs to improve the care provided to veterans.
- Music Therapy touches people in different ways. Music therapy has been found to aid in reducing pain, anxiety or restlessness; improve blood pressure; increased communication, and offers support through grief and loss. A board-certified music therapist evaluates patients and develops goals for most effective techniques.
- Massage Therapy is a holistic therapy to relieve stress, stimulate circulation and improve range of motion. Licensed massage therapists bring gentle pain and symptom relief. The goal is to bring patients greater comfort and relaxation.
- Healing Touch reduces pain, anxiety and stress; enhances emotional and mental health; and comforts at end-of-life. Certified healing touch volunteers use gentle touch to influence and support the energy system within and around the physical body.
- Pet Therapy can be very effective. Cuddling or petting a certified therapy dog can offer benefits such as lowered blood pressure, reduced anxiety, boredom, and improved mental health.
- Recordable Books is a gift that families can hold onto forever. The comfort of a familiar voice reading a favorite story. Recordable, audible storybooks capture the voice and spirit of a hospice patient, leaving loved ones with a forever keepsake.

CentraCare Health Hospice and CentraCare Health - Sauk Centre are licensed by the state of Minnesota and certified by Medicare. CentraCare Health Home Care also is accredited by the Joint Commission on Accreditation of Health Care Organizations. These regulatory agencies provide assurance that you will receive the highest quality care available.

- Help reduce financial stress.
- Provide quality educational resources on death and dying and grief and bereavement.
- Expand hospice programs such as: Bouquets with Blessings, Peaceful Moments and We Honor Veterans.
- Allow staff to receive additional training, education and development on end-of-life topics and issues.
