Becoming Our Patient
To become a patient at Coborn Cancer Center, you will need:
- A referral from your provider
- If you are not a CentraCare Health patient, your medical records will need to be sent to Coborn Cancer Center
- Medical Oncology fax: 320-200-3252

Prepare For Your First Visit
We want your visit to be safe, comfortable and as pleasant as possible. Please review the letter below for your type of clinic visit.
What to bring
- Health history form (pdf) and list of medications, herbs and vitamin supplements you are taking or complete the questionnaire in MyChart
- Your schedule for planning future appointments
- Current insurance card(s) and photo ID
- X-ray films or CDs of scans related to your visit that were not completed by CentraCare Health
- A list of questions you have for your provider
- Music, reading or other activity you can do quietly for a few hours
- Family member or friend to offer support and/or drive you home
Please leave valuables, like money, credit cards and jewelry, at home.
For your convenience, we provide TV, DVD players and laptops with internet access. Our lobby provides a space to relax, have complimentary refreshments and connect with others. Pagers are available for patients to confidently explore our campus and Coborn Healing Center while waiting, knowing when the pager alerts it is time to return to Coborn Cancer Center.
If it has been more than 6 months, please complete a dental examination and cleaning. If your dentist recommends major dental work, please first discuss it with your cancer center physician.
Health Care Directive & Advance Care Planning
Who will speak for you if you can't speak for yourself?
A health care directive is a planning tool used to identify who you want to make health care decisions for you if you aren’t able to communicate. It puts your wishes regarding end-of-life care in writing. For more information, please ask your nurse for a packet or a brochure on Health Care Directives
Local organization, Light the Legacy, provides information about Health Care Directives, as well as forms you can download and complete.

Instructions for Family & Friends
While you are waiting, we would like you and your family and friends to remain on the CentraCare Health Plaza grounds.
While waiting, you can:
- Visit Coborn Healing Center.
- Relax in a nearby waiting room.
- Grab a snack or small meal at the Woodlands Bistro.
- Use the Interfaith Meditation Room to think, rest or pray.
- Pick up prescriptions and other medicine at CentraCare Pharmacy.
- Find crossword puzzles, games and other activities at the Gift Gallery.
- Walk the 1.1-mile outdoor path around the CentraCare Health Plaza.

Oncology rehabilitation describes a wide range of therapies designed to help you build strength and endurance, regain independence, reduce stress and maintain the energy you need to participate in daily activities that are important to you.
