CentraCare Health – Paynesville Koronis Manor Care Center, attached to the main campus of CentraCare Health – Paynesville, is a 52-bed skilled, nursing home (long-term care facility). Since 1964 our compassionate, well trained staff has taken great pride in the care and respect they provide to each resident. Our care encompasses professionals from all areas of our health care organization which provide for the social, physical, recreational and religious needs of each individual. Physical therapy is available 7 days per week.
Accommodations
Caring for you
Our certified and licensed nursing staff, including RN’s, LPN’s and CNA’s, care for long-term, respite care residents and short-term rehab patients 24 hours a day. You can relax knowing that the staff caring for you is capable of handling the highest level of care, including Alzheimer’s and Dementia. Each resident’s individual plan of care is developed and carried out by an interdisciplinary team of professionals. This team includes the following departments: Nursing, Dietary, Rehabilitation Services, Social Services, Activities and Environmental Services. The resident and their loved ones are an essential part of the total care team as well and we welcome your suggestions on how we can serve you better. Koronis Manor has the “wandering system” which ensures safety and security 24 hours a day for residents.
Physicians and Medical Support
CentraCare Health – Paynesville Koronis Manor Care Center has a Medical Director, who oversees the implementation of the resident care policies and the coordination of medical care provided by the facility. After admission, your personal physician will continue to see you every 30 days for the first 3 months (if on Medicare coverage) and at least once every 60 days thereafter. These medical visits may alternate between your Physician and the Nurse Practitioner or Physician Assistant who works with your provider. An important part of your care is making decisions. Your physician will meet with you or your representative to discuss your diagnosis, prognosis, treatments, alternatives and risks.
Social Services
Another important resource is our Social Services Department, who are here to assist you and your family with the admission’s process, provide information and support in handling your adjustment, family adjustment, emotional stress, financial issues, and other areas of concern that may affect your psychosocial well being. Social Services will also work with you to inform you of your legal rights and act as an advocate and liaison for you within the facility.
Registered Dietitian
A registered dietitian oversees all menu planning to ensure proper nutrition and to make every attempt to accommodate the special diets and personal tastes of each resident.
Rehabilitation Services
Rehabilitation Services includes Physical Therapy, Occupational Therapy and Speech/ Language Pathology Services which are available to residents on an inpatient or outpatient basis. The desired goal is for each resident to achieve his or her maximum potential while residing in our care.
Pharmaceutical and Medication Management Services
Pharmaceutical and Medication Management Services are another important role in resident care. CentraCare Health – Paynesville utilizes a 30-day unit dose system of medication administration. For your convenience, CentraCare Health – Paynesville Pharmacy Services is available to you, and is the primary provider of medications for all of it’s long-term care facilities. Of course, you have the right to choose the pharmacy of your preference, provided that the chosen pharmacy will package medications in the 30-day Unit Dose System and medications are delivered to the care facility when needed. Our pharmacy staff is here to make your transition a pleasant one, and will be happy to answer your questions.
Staying Active and Involved
Remaining active is a key element to your happiness and overall well-being. Our activity staff plans and coordinates events on a weekly and monthly basis. Our many dedicated volunteers help to provide entertainment, games, music and community outings that will keep your life full and satisfying. Staff members work diligently to meet the emotional and therapeutic recreation needs of each resident, such as exercise routines and special seasonal events, such as the Let’s Go Fishing program and summer picnics at Lake Koronis. The Ministerial Association of the Paynesville area arranges and provides regularly scheduled religious services for those who wish to attend.
Meditation Chapel and Garden
CentraCare Health – Paynesville Koronis Manor Care Center is connected to a beautiful, outdoor Meditation Garden that provides our residents with a place to enjoy the outdoors. Colorful plants, running waterfalls and abundant sunshine make for an enjoyable visit on our main campus. The Meditation Chapel, part of the garden area, is a quiet, reflective space that is non-denominational and open to all.
Financial Considerations
The daily room rates for CentraCare Health – Paynesville Koronis Manor Care Center are determined by the State of Minnesota and will vary based on private/ semi-private rooms and each individuals care needs. Private payment, Medicaid (Medical Assistance), and Medicare are all accepted payment methods. The daily rate includes room, board, nursing care, activities, personal and custodial care, housekeeping, laundry and social services. We work hard to exceed your expectations and make every effort to accommodate your needs.
