CentraCare Health – Monticello Care Center is a skilled nursing facility located on the banks of the Mississippi River. The care center provides 24-hour nursing care to long-term care residents and rehab residents.

Therapeutic Recreation
Therapeutic Recreation is designed to provide each resident with the opportunity to participate regularly in their favorite pastimes and hobbies. Participants are encouraged to live life to the fullest extent in a peaceful and friendly environment. The goal of the staff is to provide for the physical, social, spiritual, intellectual and psychological needs of the residents. This is accomplished by incorporating trips, activities, spiritual programs and community events based on the requests and interests of all residents.
Social Services
The Social Services department plays an integral role in the comfort and happiness of our residents. Our full-time licensed Social Worker ensures that each resident receives optimum care based on his or her individual needs. From the moment the registration process begins, our social worker is there to ease the process for the new resident and their family. Social services continues providing assessments of each resident’s needs and recommending educational opportunities for both residents and their families. Team conferences, facilitated by the Social Services department, give residents and their families a chance to meet and discuss any concerns they have.
CentraCare Health - Monticello Care Center is fully licensed by the Minnesota Department of Health and is surveyed on an annual basis.
