CentraCare Health clinics receive Minnesota Bridges to Excellence Awards

Twelve CentraCare Health clinics will receive awards from the Minnesota Bridges to Excellence (MNBTE) program on July 27 at the Hyatt Regency Hotel in Bloomington, Minn.

These clinics will be recognized for improving the care provided to patients. They have focused on achieving an increased rate of optimal care for preventative services and chronic diseases such as asthma, diabetes, vascular disease, depression and hypertension.

The data collected is publicly reported through Minnesota Community Measurements (MNCM). The MNBTE program reviews this data to recognize improved performance at the clinic level and awards accordingly. CentraCare Health clinics receiving awards are as follows:

Becker Family Medicine: Vascular Improvement Award

CentraCare Clinic Endocrinology: Diabetes Improvement Award

CentraCare Health Plaza Family Medicine: Vascular Improvement Award

CentraCare Health Big Lake: Vascular Improvement Award and Depression Remission Improvement Award

CentraCare Health - Long Prairie: Diabetes Improvement Award and Vascular Improvement Award

CentraCare Health - Paynesville: Depression Remission Improvement Award

CentraCare Health - Paynesville-Belgrade: Diabetes Improvement Award

CentraCare Health - Paynesville-Cold Spring: Vascular Improvement Award

CentraCare Health - Paynesville-Eden Valley: Diabetes Improvement Award and Vascular Improvement Award

CentraCare Health - Paynesville-Richmond: Diabetes Improvement Award and Depression Remission Improvement Award
