CentraCare Health Receives Grant to Improve Diabetes Care for Diverse Patients

St. Cloud, Minn. – CentraCare Health recently received a $370,000 Accountable for Community Health grant from the Minnesota Department of Health. This two-year grant will help CentraCare improve diabetes care and outcomes for Hispanic and East African populations in Stearns County, enhance patient satisfaction and patient experience, reduce disparities in care and make more community resources available to patients.

CentraCare will partner with local organizations and businesses, including Great River Interfaith Partners/Isaiah, Hands Across the World, Create CommUNITY, Casa Guadalupe, Lutheran Social Services, African Women’s Alliance, St. Cloud Area Somali Salvation, Electrolux, Jennie-O and Gold’n Plump, to increase awareness of diabetes and develop ongoing relationships with diverse populations.

Eunice Adjei-Gyimah was hired as the project manager and will collaborate with two CentraCare community health workers to be hired this summer. Before joining CentraCare, Eunice Adjei-Gyimah was coordinator of Parents in Action at St. Cloud School District 742. She is chair of the St. Cloud Regional Human Rights Commission and is on the board of Tri-CAP, African Women's Alliance, Midwest Oromo Communities and the St. Cloud State Alumni Association. She was honored in 2012 as one of the Ten Young outstanding Minnesotans by the Jaycees, was featured in the Central Minnesota Women magazine and received the Rock-on Award for Civic Engagement from the mayor of St. Cloud. She’s a Ghana native who immigrated to St. Cloud in 2004 and always knew she wanted to represent under-served populations. Eunice is committed to helping others understand cultural barriers, defining workplace diversity and coaching others in how to communicate with clients of color. She holds a bachelor’s degree in Psychology and master’s degree in Social Responsibility from St. Cloud State University.
